MUSIC AWARD APPLICATION
This award is to encourage musical interests among Jamestown residents as well as non-residents who are active participants in the Jamestown music community. It’s funded by The Rhode Island Foundation through the Jamestown Community Fund and the members of the Jamestown Community Chorus. The award is given annually at our May concerts. Applications are usually due by the beginning of April. Call 401-423-1574 for specific due dates.
The annual one-time cash award is for up to $500 and can be used for, but is not limited to: instructional books, musical lessons, music workshops, undergraduate education, and/or expenses related to music development.
Applicants must:
· be of middle school age or older (grade 6 and up)
· be a full-time resident of Jamestown or a non-resident who actively participates in the Jamestown music community
· use the award within one year
· complete the application process below
· be available to attend the Sunday, May 3, 2009 concert of the Jamestown Community Chorus, where the prize will be awarded
· complete an essay of how the prize will be used
· submit a letter of reference from someone who is aware of your music plans.
Application Process:
To apply, please provide the following information, answer the following questions, and send to the above address. (Application must be postmarked no later than March 27, 2009.)
1. Your name – address - phone
2. Name of school and grade (if student)
3. Letter of reference (other than immediate family)
4. Submit a well-written essay that includes the following:
· your musical background
· how you will use the award, specifying teachers, institutions, workshops, lessons, materials, and the like
· how this award will further your musical interests
5. Sign and date. (If applicant is under 18 years of age, parent or guardian must sign as well.)
